10.4  Pathology of the foreskin, phimosis, balanitis

10.4.1  Balanitis

Clinical signs:

  • inflammation of the prepuce
    • acute inflammation (bacteria, mycotic)
    • chronic inflammation of uncertain etiology (balanitis plasmocellularis Zoon)
    • lichen sclerosus et atrophicus
  • condylomata accuminata
10.4.1.1  Balanitis plasmocellularis Zoon

Histology:

Balanitis (and vulvitis) plasmocellularis of Zoon is the form of a chronic inflammation of mucosa, characterized by variable plasmocytic infiltrate, slight regular spongiosis of the epithelium and variable hemorrhage with siderophages.

10.4.1.2  Balanitis circinata

Clinical signs:

  • ulcers of irregular, sharply defined outline on glans
  • similar vulvitis also occurs
  • age: usually children
  • associatiation: Reiter syndrome (arthritis, conjunctivitis, urethritis)

Histology:

Resembles psoriasis: spongiform pustules early, later acanthosis, hyperkeratosis, parakeratosis.

10.4.2  Phimosis, paraphimosis, frenulum breve and other lesions of the penis

Clinical signs:

  • Phimosis: inability to retract the prepuce (congenital, postinflammatory); complicatons:
    • accumulation of the smegma
    • chronic and acute inflammation
  • Paraphimosis: strangulation of the penis in patients with phimosis after forced retraction of the prepuce
